MEDICAL FORMS

All students and adults meeting at the church campus and traveling away for a Student Ministry event must have a medical information form on file with the Student Ministry office. Just one form per school year (August-August) is needed.

The PDF version of the form can be printed and filled out and singed by hand, or the Word Document version can be filled out and signed electronically. Either option is acceptable and can be clicked on below. Forms can be turned in to the office or emailed to studentministry@stpkaty.org.
